Regarding issuance of health examination スポーツくじ

Dear Shinagawa Campus students,

Health examination certificates for 2025 can now be issued using automatic スポーツくじ issuing machines.
After undergoing the regular health checkup for students in April,Students who are subject to re-examination, students with chronic illnesses, and students who have not responded to the online questionnaire will not be issued a スポーツくじ from the automatic issuing machine, so please check at the Health Center.
*If it cannot be issued, the message "Necessary information is insufficient." will be displayed.
(As a general rule, the スポーツくじ will be issued at the Health Service Center two days after the application date (on a weekday).)
