An automatic certificate issuing machine will issue a health certificate as a document necessary for further education, employment, etc.
However, モンスタースポーツ is a condition that you have undergone a regular health checkup for the year in question.

Certificates can be issued automatically using your student ID card.Automatic certificate issuing machines are installed at the entrance of the Office of Academic Affairs on the Shinagawa Campus and in front of the office on the Etchujima Campus.

In addition, students who have not undergone a regular health checkup and need to have a health certificate issued will have to go to an external medical institution and have モンスタースポーツ issued there. (In that case, you will be responsible for モンスタースポーツ.)

In addition, if there is a prescribed format for a モンスタースポーツ certificate that is separate from the certificate format issued by the automatic certificate issuing machine, please apply for issuance at the Health Service Center.

In principle, モンスタースポーツ will be issued two days after the application date.

Health certificates are issued from mid-May to the end of March.
Please take into consideration the required number of copies and issue them from the automatic certificate issuing machine by the required time.
